AUTHORIZATION & CONSENT

As a parent it's important that you let us know how you would like us to help you manage the current crisis being experienced in your home. Please fill out the following questions indicating your preferences, authorization and consent. 

Is your child using drugs, self-harming or suicidal?
Have the police or social services been involved?

LEGAL & DISCLAIMER: Please note, Crisis Care is only available to current clients who have signed the Healthy Routes Consent and Liability Contract, for which all appendixes apply. Crisis Care has limitations, we refuse the right to work in homes where there is active violence or substance abuse (we recommend having the police or other professional services intervene if necessary) as well as tolerate any threats or intimidating behaviour from our clients or their families. In instances where self-harm, harm to others or suicidal ideation presents itself Healthy Routes recommends that the child be taken to CHEO Emergency for an evaluation and reserves the right to make time-sensitive arrangements on behalf of the child's (clients) wellbeing where necessary. Crisis Care is available between the hours of 7am to 7pm, during which time we will work with the client and parent(s) to reduce, mitigate and resolve the impact of the situation, circumstance or crisis [as per the selected level of Care you choose to invest in] until we can resume normal sessions or find appropriate resources. All costs associated with Crisis Care are non-refundable [unless we are unable to accept them as a Crisis Care Client at this time] and services end within a 14 day period, after which further sessions are available (moving clients into maintenance mode).
