Healthy Routes takes pride in valuing the relationships with our clients and believes that such a relationship is the beacon in the healing process. Because each individual is unique, we believe in a wellness model that helps our clients empower themselves by focusing on what works for them. One's journey is not the same as the other.

The following Terms & Conditions match those found during your original Client Intake process, with some updates where eligible. Please review and read these conditions carefully to ensure that we are all, always, on the same page with each other and our responsibility to each other.

Nature of Counselling and Coaching Sessions
We use various therapies to help support our clients to meet their needs.
SOLUTION-FOCUSED THERAPY is viewed as a goal-directed approach. The focus is on the client’s health rather than the problem, drawing from strengths rather than weaknesses and providing coping strategies that help
reach future goals.
COGNITIVE-BEHAVIOURAL THERAPY can help people find ways to behave by changing their thinking patterns to help reduce stress, cope with complicated relationships, deal with grief, and face many other common life challenges.
ACCEPTANCE AND COMMITMENT THERAPY encourages people to embrace their thoughts and feelings rather than fight them. It can be paired well with Mindfulness-Based Cognitive Therapy, which involves a combination of cognitive therapy, meditation, and a non-judgmental attitude called mindfulness which can provide effective treatment.
WALK THERAPY is a form of psychotherapy that incorporates walking while talking about issues and problem-solving.
During our sessions, one or more of these therapies may be used and this is entirely dependent on what is
needed in your healing process.

Relationship
Healthy Routes takes on unconditional positive regard for all their clients and greatly believes in the importance of the therapeutic relationship. Through our sessions together, it is important that our clients grow and become the version they seek. It may feel uncomfortable; our goal together is to make it comfortable. The results of this depend on the willingness to engage in this process with an open mind; understand that there may be moments of discomfort, and emotional upheaval, which is what the healing process is all about.
The first few sessions will involve an evaluation of your needs and a suggested plan. We strive to get our clients on the right path with a vision in mind. Counselling involves a personal commitment to oneself, time and energy.
Change is a process.
Healthy Routes promises to support, listen and provide a safe and comfortable setting during all our sessions to allow room to grow and discover oneself. It is important for you to understand that you are
fully responsible for your well-being both during and after the sessions, including the choices and decisions you make. As our sessions may take place in your home and not in a clinical setting, it is important that
you disclose to me in advance if any conditions in your home may pose a threat to your or my health or safety.

Disclaimer & Responsibility
Please also understand that the services offered are therapeutic/coaching and are NOT a substitute for clinical mental health care or medical care and are NOT intended to diagnose, treat, or cure any mental health or
medical conditions.
By agreeing to the Terms & Conditions, I understand that Healthy Routes and, more specifically, Jennifer Avery is not acting as a medical professional. The services offered by Healthy Routes are not a replacement for medical, psychological, psychiatric treatment or other treatments you may require from a licensed health professional.

Confidentiality
We highly respect the privacy and confidentiality of each of our clients. Any notes taken by the counsellor/coach during counselling shall be kept confidential and secure by the counsellor/coach and shall not be disclosed to anyone without prior written consent by the client/patient. We maintain our records for seven (7) years, after which they are destroyed.
However, our client must understand that privacy has its limitations in law, and there are circumstances in which we must share information with a third party(parent, legal guardian, law enforcement, Children’s Aid Society) without your consent.
â–ª Acts of sexual abuse or misconduct
â–ª Criminal acts
â–ª Acts of abuse towards others, such as neglect towards children, the disabled, or the elderly
â–ª Acts that the therapist believes may cause harm to themself or others.

Sessions and Fees
An invoice will be mailed to you on the day of each session from a third-party app called Square. Payment can be made directly through the invoice, following the prompts. It is a straightforward, safe and easy-to-use system.
Fees are to be paid at the time of each session.
Refusal or failure of payment will result in possible termination of coaching/counselling unless alternate arrangements have been made.
PLEASE NOTE: These prices are accurate as of the date of this contract but may be applicable to change in the future.

Cancellations
At Healthy Routes, we understand that you may periodically need to reschedule your session(s) with today's busy lifestyle. To ensure that we make the best use of your time and others, it is imperative should you need to reschedule a session; we require a 24-hour written notice to support@healthyroutes.ca.
Failure to give appropriate notice will result in the full fee being charged for the session.

TELE-MENTAL HEALTH INFORMED CONSENT
I understand that telemental health is the practice of delivering clinical health care services via technology-assisted media or other electronic means between a practitioner and a client located in two different locations.
1) I understand that I have the right to withdraw consent at any time without affecting my right to future care, services, or program benefits to which I would otherwise be entitled.
2) I understand that there are risks, benefits, and consequences associated with telemental health, including but not limited to disruption of transmission by technology failures, interruption and/or breaches of confidentiality by unauthorized persons, and/or limited ability to respond to emergencies.
3) I understand that there will be no recording of any of the online sessions by either party. All information disclosed within sessions and written records pertaining to those sessions are confidential and may not be disclosed to anyone without written authorization except where the disclosure is permitted and/or required by law.
4) I understand that the privacy laws that protect the confidentiality of my protected health information (PHI) also apply to telemental health unless an exception to confidentiality applies (i.e. mandatory reporting of a child, elder, or vulnerable adult abuse; danger to self or others; I raise mental/emotional
health as an issue in a legal proceeding).
5) I understand that if I am having suicidal or homicidal thoughts, actively experiencing psychotic symptoms or experiencing a mental health crisis that cannot be resolved remotely, it may be determined that telemental health services are not appropriate and a higher level of care is required.
6) I understand that during a telemental health session, we could encounter technical difficulties resulting in service interruptions. If this occurs, end and restart the session. If we cannot reconnect within ten minutes, please call me at 613 293 3392 to finish the call or reschedule.
EMERGENCY PROTOCOLS
I need to know your location in case of an emergency. You agree to inform me of the address where you are at the beginning of each session. I also need a contact person who I may contact on your behalf in a life- threatening emergency only. This person will only be contacted to go to your location or take you to the hospital in the event of an emergency.
Where applicable: I have read the information provided above and discussed it with my therapist. I understand the information contained in this form, and all of my questions have been answered to my satisfaction.

Communication Outside of Counselling and Coaching
Healthy Routes' form of communication is by email or voicemail.
Please remember that email and voicemail messages may not always be secure mediums.
We encourage you not to email or leave a voicemail regarding personal information; please wait to speak to someone directly about your concern.
Call 911 to access the local emergency response system or go to the nearest emergency room if an emergency is particularly life-threatening.
We are NOT a hotline crisis center.

Healthy Routes will make every effort to return your message within 48 hours of receiving it. Calls and emails after 5 pm or on weekends and holidays will be returned on the following business day.

Termination and Clients Rights
In addition to confidentiality, you have the right to end therapy for whatever reason, without any legal or financial obligation, except for the fees already incurred.
Healthy Routes’ also reserves the right to temporarily pause or end therapy as may be determined at our sole discretion.

Release and Waiver Liability
By agreeing to the Terms & Conditions you are hereby releasing and forever discharging Healthy Routes and Jennifer Avery and their heirs, beneficiaries, executors, successors and assigns, directors, officers and shareholders from any and all actions, cause of actions, claims and demands for damages, compensation, loss or injury, howsoever arising, which heretofore may have been or may hereafter be sustained by you arising from or as a result of any advice given or otherwise resulting from the therapeutic/coaching relationship contemplated by this Agreement.
Except as otherwise provided in this Agreement, the invalidity or unenforceability of any term of this Agreement does not affect the validity or enforceability of any other term. Any invalid term will be treated as severed from the remaining terms.
This Agreement shall be governed and constructed in accordance with the laws of the Province of Ontario and the laws of Canada applicable therein.
This Agreement and the rights and obligations provided herein shall be binding upon the successor, assigns, heirs, executors and administers of the parties Except as otherwise expressly provided herein, the provisions
hereof shall inure to the benefit of, and be binding upon, the successors, assigns, heirs, executors and administrators of the parties hereto.
By agreeing to the Terms & Conditions you indicate that you have read and understand the information noted above. Please note: these Terms and Conditions are the same as those in your initial Contract, when signing on as a new client with Healthy Routes.
